THIS AGREEMENT CONTAINS PROVISIONS THAT LIMIT OUR LIABILITY TO YOU AND REQUIRE YOU TO RESOLVE ANY DISPUTE WITH US THROUGH FINAL AND BINDING ARBITRATION ON AN INDIVIDUAL BASIS, AND NOT BY A LAWSUIT OR RESORT TO COURT PROCESS.

• "Services" means all wellness services and testing coordination provided by Neuraris

• "Testing Partners" means third-party laboratories and testing facilities

• "User" or "you" means any person accessing the Platform or Services

• "Wellness Information" means non-medical educational content and recommendations

• "Content" means all information, data, text, and materials on the Platform

• "Biological Samples" means any physical specimens submitted for testing

B. Important Notices and Disclaimers

NEURARIS IS NOT A HEALTHCARE PROVIDER. Neuraris provides wellness services and facilitates access to third-party testing services. We do not provide medical advice, diagnosis, or treatment. The decision to use any information or pursue any testing offered through our Platform rests solely with you.

THE INFORMATION AND SERVICES AVAILABLE ON OR THROUGH OUR PLATFORM ARE FOR WELLNESS AND EDUCATIONAL PURPOSES ONLY AND ARE NOT INTENDED TO SUBSTITUTE FOR PROFESSIONAL MEDICAL ADVICE, DIAGNOSIS, OR TREATMENT. Always consult with your healthcare provider regarding questions about any medical condition or before making health care decisions.

2. No Insurance Accepted; Notice of Financial Responsibility

Neuraris does not accept or process insurance claims. Our Services and those of our Testing Partners are provided on a direct-pay basis only. We and our Testing Partners do not participate in or bill any commercial health insurance plans and are not enrolled with any federal or state healthcare programs such as Medicare or Medicaid.

By choosing to use our Services, you:

• Agree to pay directly for all Services

• Acknowledge that Services may not be covered by your insurance

• Understand that we will not submit insurance claims on your behalf

• Accept full financial responsibility for all charges

• Agree not to submit claims to federal healthcare programs

• Understand that Testing Partners may have separate billing policies

A. Service Overview

Our Platform allows users to:

• Purchase and coordinate third-party testing services

• Access non-medical cognitive assessments

B. Testing Services

Testing services include:

• Dried blood spot testing

C. Service Limitations

Our services explicitly DO NOT include:

6. Payment Terms

• Pay all fees and applicable taxes

• Accept Stripe's terms of service

B. Third-Party Financing

We may offer financing options through partners such as Paybright or Affirm ("Finance Partners"). If you choose financing:

• You enter a direct agreement with the Finance Partner

• Their terms and conditions apply

• We are not responsible for financing disputes

• Approval is at Finance Partner's discretion

C. Refunds and Cancellations

Our refund policy:

• Unused test kits may be refundable within 30 days

• Processing fees are non-refundable

• Shipping costs are non-refundable

• Used test kits are non-refundable

• Service fees are non-refundable

• Testing Partner fees may be non-refundable

7. Third-Party Testing Partners and Disclaimers

A. Testing Partner Relationship

Our Testing Partners are independent laboratories and facilities that:

• Maintain their own licenses and certifications

• Set their own testing protocols and standards

• Have separate terms of service and privacy policies

• Process and analyze all biological samples

• Generate and validate all test results

• Maintain direct responsibility for testing accuracy

B. Testing Disclaimers

You explicitly acknowledge and agree that:

• All testing is performed solely by Testing Partners

• Neuraris does not process any biological samples

• We cannot guarantee testing accuracy or turnaround times

• Testing Partners may reject unsuitable samples

• Results may be delayed or inconclusive

• Test results are not diagnostic or medical in nature

• Testing availability may vary by jurisdiction

• Additional Testing Partner terms may apply

C. Sample Collection and Handling

For biological sample collection:

• You must follow all provided instructions exactly

• Improper collection may invalidate results

• You are responsible for proper packaging

• Lost or damaged samples are not our responsibility

• Some samples may require special handling

• Testing Partners may have specific requirements

8. Mobile Applications

A. Apple App Store Terms

If you download our app through Apple's App Store:

• These Terms are between you and Neuraris only

• Apple is not responsible for the app or support

• Apple has no warranty obligation

• Apple is not responsible for claims or liability

• You must comply with App Store terms

• Apple may enforce these Terms as a third party

B. Google Play Store Terms

If you download our app through Google Play:

• These Terms are between you and Neuraris only

• Google is only the app provider

• You must comply with Play Store terms

• Google has no liability or obligation

• Google is a third-party beneficiary

14. Term and Termination

A. Term

• Continues until terminated
